  • Via bank account: download this Download file Authorization Agreement for direct deposit form to begin automatic withdrawals from your bank account using Automated Clearing House (ACH). You can specify the date of the withdrawal and you will receive a receipt from CMDA each month.

Commemorative Gifts
Contributions in honor of friends and colleagues are a wonderful way to mark anniversaries and celebrate milestones. Gifts in memory of loved ones pay lasting respect by advancing the Lord’s work. Both are gratefully received and thoughtfully acknowledged. When making commemorative gifts, donors are asked to send the names and addresses of those who should be notified about the gift to:

Christian Medical & Dental Associations

Donor Advised Funds
Donor Advised Funds offers the opportunity to donate cash, stock or other approved assets to a special charitable account, claim a charitable deduction to save taxes and then later recommend the ministries and charities to support. Consider this effective family foundation substitute for gifts of $10,000 or more. CMDA has a relationship with the National Christian Foundation to accommodate donors who wish to create a Donor Advised Fund to provide gifts to the various ministries they support. For more information, please contact CMDA's Stewardship Department.

Gold and Silver
With precious metals and historic highs, you can avoid capital gains tax (which can run as high as 28 percent for coins and collectibles) by contributing gold and silver to CMDA. Contact CMDA's Stewardship Department to learn more.

Heartchangers

Monthly update letters are sent out to Heartchangers, our regular financial supporters who are dedicated to advancing the mission of changing hearts in healthcare through financial support. Leaving A Legacy

A gift by will benefits the Christian Medical & Dental Associations and may provide estate tax savings. The bequest may consist of cash, securities, real or personal property. It may be made outright or in trust.

There are three basic types of bequests -- a specific bequest; a residual bequest, which leaves the remainder of one’s estate after debts, taxes, expenses and other bequests; and a contingent bequest, which provides for a contingent beneficiary if one’s first-named beneficiary(ies) does/do not survive. Sample will language for each type follows:

Specific Bequest

“I give, devise and bequeath all my right, title, and interest in and to (describe the specific property – real estate, personal property, securities, IRAs, etc.) to the Christian Medical & Dental Society (dba Christian Medical & Dental Associations) (Federal Tax ID # 36-2284267), P.O. Box 7500, Bristol, TN 37621”

or

“I give, devise and bequeath (insert dollar amount or percent of estate) to the Christian Medical & Dental Society (dba Christian Medical & Dental Associations) (Federal Tax ID # 36-2284267), P.O. Box 7500, Bristol, TN 37621.”

Residual Bequest

“I give, devise and bequeath my residuary estate, which is all the rest, residue, and remainder of my property, real and personal of every kind and description and wherever located (including all legacies and devises that may for any reason fail to take effect), belonging to me at the time of my death or subject to my disposal by will, to the Christian Medical & Dental Society (dba Christian Medical & Dental Associations) (Federal Tax ID # 36-2284267), P.O. Box 7500, Bristol, TN 37621.”

Contingent Bequest

“If the above-named beneficiary predeceases me, then I give, devise and bequeath such amounts or property absolutely to the Christian Medical & Dental Society (dba Christian Medical & Dental Associations) (Federal Tax ID # 36-2284267), P.O. Box 7500, Bristol, TN 37621.”

Bequest Intent and Suggested Language

The following language should be used to describe the intent of an unrestricted bequest, to be used for the greatest needs of CMDA:

“Said (describe specific property, or insert dollar amount or percentage) is to be placed in the General Fund to benefit the Christian Medical & Dental Associations.”

The following language should be used to describe the specific intent of a designated bequest, to ensure that all parties involved in the bequest clearly understand the donor’s intent:

“Said (describe specific property, or insert dollar amount or percentage) is to be used to benefit [insert name(s) of specific ministry(ies), program(s) or initiative(s)].”
