 Missionary/Resident/Retired & Spouse - $350.00

 Allied Health Professional & Spouse - $350.00
 Allied Health Professional & Spouse - $245.00
 Student & Spouse - $100.00

Continuing Education Fee - $55.00 for 4.0 Hours


Late Registration Fee - Add an additional $25.00 per person to the registration fee after April 13, 2012.


Non-member Fee - Add an additional $100.00 per registration to the total registration fee.


Children's Registration - Registration for the first child is $100.00 and $80.00 for each additional child.


Cancellation Fee - Cancellations and requests for refund must be submitted in writing to the Christian Medical & Dental Associations.
Cancellation charges will be assessed as follows:
Before or on April 13, 2012 - $50.00/person
After April 13, 2012 - No refund
